I acknowledge that I have knowingly and voluntarily elected to proceed with Colour Treatment Services including but not limited to Men’s Colour, Women’s Colour, Partial Highlights, Full Highlights, Balayage, Colour Correction, Perms, Keratin Treatment as well as any additional chemical treatment at TopnotchBarber. As a part of this process, I acknowledge that I have read and understood the terms and conditions as outlined below:

• I must provide TopnotchBarber with a forty-eight (48) hour notice in the event of a cancellation or change in appointment time. If a forty-eight (48) hour notice is not provided, I acknowledge that I will be subject to a 50% charge of the service fee.
• I agree that a fifty percent (50%) deposit is required to secure an appointment timeslot at TopnotchBarber. All deposits are non-refundable. Upon completion of the service, I agree that I must pay the remainder of the balance of the service received.
• I must inform TopnotchBarber, prior to any treatment received, of any prior medical history (both physical and mental), allergies, ailments, and/or any other conditions which would otherwise prevent the colour technician and hairstylist from safely completing the service.
• I agree that TopnotchBarber is permitted to utilize images taken of the service(s) rendered for any marketing purposes which can include social media and/or our website.
• I agree due to the history and condition of my hair, that TopnotchBarber cannot honor any guarantee. However, TopnotchBarber can perform any adjustments for a charge.
• I release, TopnotchBarber and their Stylist from any responsibility and/or liability concerning the application, processing and/or consequences of the chemical color procedure of my hair. I consent to have TopnotchBarber conduct services to my hair including a chemical process being applied to my hair.
• I release, forever discharge, and hold harmless TopnotchBarber, against any and all liability, damage, and/or expense arising out of or in connection with actions, claims, and/or damages resulting in personal injuries and disabilities (physical and/or psychological) that I might incur as a result of the chemical alteration through the hair colouring processing of my hair. I understand that additional conditioning treatments may be recommended and/or necessary for my hair maintenance and that permanent damage to my hair is possible due to the chemical application.
• I release, forever discharge, and hold harmless TopnotchBarber, against any and all liability, damage, and/or expense arising out of or in connection with actions, claims, and/or damages resulting in personal injuries and disabilities (physical and/or psychological) that I might incur as a result of the chemical alteration through the hair colouring processing of my hair. I understand that additional conditioning treatments may be recommended and/or necessary for my hair maintenance and that permanent damage to my hair is possible due to the chemical application.
• I declare that I have not had any chemical process performed on my hair within the last six weeks.
• Further, I hereby release, forever discharge and hold harmless TopnotchBarber, its affiliates, licenses, members, directors, officers, managers, employees, officers, advisors, assigns, agents, legal representatives and from all manner of actions, causes of action, debts, accounts, contracts, claims, and demands, and liabilities whatsoever of every name and nature, both in law and in equity, whether known or unknown, including, but not limited to, illness, paralysis, death, damages, or bodily injury, that may arise as a result of the services performed by TopnotchBarber.
